Please bring your PD5 forms if you have any questions or would like to turn the forms in. The PD5 forms and the Supplemental Income Forms were mailed January 10th and are due back by March 1st. The property owners receiving a Senior or Disabled Deduction are required by the State fill out these forms every year.
